You can make an eye appointment with us by phone or over the Internet by using the following form to send an email request to us.

Please note that the following form is used only to request an appointment. It does not actually allow you to schedule an appointment for yourself. When submitted, one of our appointment administrators will contact you to schedule an appointment.

Note 1 By filling in the email address you are giving us permission to send an appointment confirmation and reminders to you by email. These emails will contain:
  • Information on our office policies and what to bring to your appointment
  • Links that allow you to download and print various medical forms that can be completed and brought to our office the day of your appointment saving you from having to fill these out in our waiting room.
  • Buttons that allow you to Confirm or Cancel your appointment over the Internet.

Because email reminders are more reliable and less intrusive than phone reminders, we highly recommend that our patients use our email reminder service. Please see our Privacy page for more information on appointment reminders and privacy.
